  1. Please find the Board Seat and the Committee Chair report forms below.
  2. Once form(s) are filled out, use the print function to “print to .pdf” and save as a .pdf to your computer (clicking save as a .pdf does not flatten your input fields).
  3. Then, use the submission button to submit your documents. Please submit committee report and documents separate from your chair report and documents.

Event proposals should be provided to the Fundraising Chairs to be submitted with their committee report (their signature is required). Check requests for reimbursement can be submitted using the reimbursement link.
